Synthetic antioxidant actoprotector in the 3-hydroxypyridine family — sold in Russia and CIS as Mexidol and related brands, also repurposed as a neuroprotective in cognitive and anxiety indications. Do not confuse with the benzimidazole scaffold of bemitil: this is a different chemistry.

Synthetic 5-ethoxy-2-ethylthiobenzimidazole — same benzimidazole scaffold as bemitil, with 5-ethoxy substitution that shifts activity profile. Mechanism: antihypoxant + hepatoprotective + regenerative. Direct findings: (1) liver regeneration after partial hepatectomy — ethomersol (5-ethoxy-2-ethylthiobenzimidazole) comparable to bemitil (2-ethylthiobenzimidazole), both accelerate liver mass restoration, increase total nucleic acids, in combination with yakton (succinate mono-(2-dimethylamino)ethyl ester) (Gaivoronskaya-Okovityy-Kolyshev-Shustov-Bolotova 2013); (2) antihypoxic activity — 7 new 1-(tietanyl-3)2-benzimidazole derivatives tested on 4 hypoxia models, compared to bemitil and ethomersol reference; thietazole has lowest toxicity (Sadykov 2009); (3) condensed benzimidazole derivatives with antioxidant activity have antihypoxic activity (Spasov-Kosolapov-Anisimova-Zhukovskaya 2019); (4) actoprotector clinical application review — bemitil and ethomersol framed as disaster medicine, military medicine, radiation consequences (Lyazikov-Pitkevich-Melnik 2011).

Clinical indication: Never commercialised outside the USSR/RF military formulary. Animal and clinical-uncontrolled data only. PMIDs 23012992 (metaprot + ethomerzol in carbophos intoxication) confirms it was studied as a parenteral adjunct in organophosphate poisoning.
